 Introduction 

In 2006 the first edition of “Dynamic Stabilization of the lumbar spine with Dynesys®” was published. More than 60 authors from all over the world contributed to this publication with their experience of Dynesys. It represented a critical review of clinical and biomechanical data, which was collected over the years.

Collecting long-term statistics for relatively new technologies is essential to prove the clinical efficacy and longevity of these systems. This edition, which is published 20 years after the first implantation of Dynesys, continues to monitor clinical results and biomechanical data of Dynesys. The system has seen a carefully considered technical evolution over the past two decades, including the introduction of the Hydroxyapaptite coated screws, the launch of Dynesys Top Loading (DTL), specifically designed for the transmuscular interspinous approach and the addition of Dynesys Transition Option (DTO) and DTO 5.5 to address different levels of degeneration. The key biomechanical principles of Dynesys, however, remain untouched. In addition to numerous new papers that are published in this edition, some chapters in Part I of this book that cover “The Fundamentals of Dynamic Stabilization with Dynesys” were included from the first edition that was published in 2006.

To date, more than 50,000 patients have been treated with Dynesys wordwide. With over 100 publications, Dynesys is the most studied pedicle screw based posterior dynamic stabilization system. This publication presents the work of many international colleagues, who I would like to thank for their time and dedication to share their findings and experiences with Dynesys. I am delighted that I received many contributions from our colleagues in Asia, a region where posterior dynamic stabilization plays an increasingly important role.

Our understanding of posterior lumbar dynamic stabilization contributes to the evolution of Dynesys and I trust that the publications in this book provide a source of inspiration to the users of the system and those who are considering the use of dynamic stabilization for the treatment of degenerative lumbar pathologies.
